What we hear from owners and HR

Small employers face the same pressures as big companies, with fewer people to deal with them. KFF’s 2026 survey puts the average employer family premium at $28,303, up 5% in a year.

Renewal shock

Premiums jump every year, with little explanation and few alternatives offered.

Losing good hires

Candidates compare benefits, and small teams often lose out to larger employers.

One plan, many needs

A single 24-year-old and a family of five need very different coverage.

No time for admin

Owners and HR already wear many hats. Benefits shouldn’t take over the week.

Who it fits

  • Employers who can enroll at least 5 employees
  • Teams that want one shared plan and one clear set of benefits
  • Owners who want a budget they can plan around

Know before you enroll

These plans are not ACA minimum essential coverage or minimum value plans, and the integrated gap is designed to reduce eligible out-of-pocket costs, not guaranteed to. Some services need authorization, and benefits, exclusions and limits are set by the plan documents. We walk through all of it with you before you decide.

ITH SmartCare is powered by New Journē Consulting. Program information only. Objectives are not guarantees of savings or coverage.

Four ways to structure coverage

We’re independent, so we show you the options side by side and recommend what fits, not what one carrier sells. The ITH SmartCare group plan is where most teams of five or more start.

Primary option

Group list billingUnitedHealthcare · Everest · OneShare

Each employee gets their own plan, and you pay one consolidated monthly bill. Prices are set by age, so a healthy team often sees the lowest monthly cost of any option here.

These are short-term medical and health sharing options, not ACA plans. They include a health questionnaire and do not cover pre-existing conditions. OneShare is health sharing, not insurance.

Best forHealthy teams that want the lowest cost with simple admin
Not ACA coverage

ICHRASet up on the Ichraly platform

You set a monthly allowance by employee class. Each employee picks their own ACA plan from the individual market and you reimburse them, tax-free for qualified expenses. It stays employer-sponsored, and ACA protections, including pre-existing condition coverage, apply to every employee.

Best forA fixed, predictable budget with employee choice
ACA coverage

Supplemental benefitsAllstate · UnitedHealthcare · Aflac · New Era

Dental, vision, accident, disability, term life, hospital, critical illness and fixed indemnity, often offered as voluntary, employee-paid benefits. We run all of it in house and give each employee an enrollment link.

Best forRounding out any package

Availability varies by state, group size and carrier. Final benefits are set by each plan’s official documents. Need something different, such as ACA small-group coverage? Ask us what else is available for your business.

Side by side

The biggest difference is whether the coverage is ACA coverage. That affects what is covered and who can be approved.

Spotlight

ICHRA: you set the number

An Individual Coverage Health Reimbursement Arrangement lets you give employees a set amount for health insurance. Each person chooses their own ACA plan, and you still offer it as an employer benefit.

  1. You choose a monthly allowance for each employee class, such as full-time and part-time.
  2. Each employee picks an ACA plan that fits their family, doctors and budget.
  3. You reimburse premiums up to the allowance. Your cost doesn’t change when a carrier raises rates.

List billing, explained

One bill for the business, with each employee on the plan that fits them. It is the most common route for healthy teams that want to keep costs down.

Everest

Short-term medical with a deductible you choose.

  • Pays 80% after the deductible, with a $5,000 out-of-pocket maximum
  • Doctor and specialist copays
  • Nationwide Prime Health Services PPO network

UnitedHealthcare TriTerm

One application covers up to 36 months.

  • Plan pays 70% after the deductible
  • Copays for office, preventive and urgent care
  • UnitedHealthcare Choice network

OneShare Health

Health sharing, where members share eligible bills.

  • Sharing amount applies before bills are shared
  • Not insurance, and sharing is not guaranteed
  • Waiting periods apply

Plan details, prices and availability vary by state, age and carrier, and are confirmed at application. Short-term medical plans do not cover pre-existing conditions and are not ACA coverage.

Build the package in layers

Many employers pay for core medical and offer the other layers as voluntary benefits. Your team gets more, and your cost stays where you set it.

Supplemental and fixed indemnity plans pay set benefits and do not replace major medical coverage.

Dental and vision

Low-cost benefits employees use and notice all year, through Allstate and UnitedHealthcare.

Supplemental protection

Accident, disability, term life, hospital, critical illness and fixed indemnity through Allstate, UnitedHealthcare, Aflac and New Era.

Common questions

How long does it take to set up group coverage?

Typically about 30 to 45 days before the start date, depending on the carrier, plan type and how quickly we receive your census and employer documents. Starting early protects the start date you want.

Is there a minimum number of employees?

It depends on the structure. The ITH SmartCare group plan requires a minimum of 5 enrollments. List billing and ICHRA can work for smaller teams. We’ll tell you which options your team qualifies for.

What is the ITH SmartCare group plan?

It is a small-group major medical program with three plan levels, Access Value, Protect and Premier, on the Cigna network. An integrated gap design is intended to reduce eligible out-of-pocket costs, and the plan documents govern what is covered. It is not ACA minimum essential coverage or a minimum value plan, so we walk through what that means for your business before you decide.

Does it cost anything to work with you?

No. Our review and quotes are free. Brokers are generally paid by the insurance carriers. ICHRA administration platforms may charge a separate monthly fee, and we’ll show you any fees up front.

What is the difference between list billing and ACA coverage?

List billing options like Everest and UnitedHealthcare short-term medical are typically lower cost for healthy teams, but they use a health questionnaire and exclude pre-existing conditions. ACA coverage through an ICHRA covers pre-existing conditions and is available regardless of health. We’ll help you decide which fits your team.

Can we add dental, vision, disability or life insurance?

Yes. We offer Allstate and UnitedHealthcare dental and vision, accident, disability and term life coverage, alongside any medical option. We handle it in house, and each employee gets an enrollment link.

Can we keep our current plan and just add benefits?

Yes. Many employers keep their medical plan and add dental, vision or supplemental coverage. We can also shop your current plan at renewal.

Can I offer these benefits to 1099 contractors?

Group plans and ICHRAs are for W-2 employees. Contractors can still get individual coverage through us, and we’re happy to help them directly.

Is health sharing the same as insurance?

No. Health sharing programs are not insurance and are not regulated as insurance. We explain the differences clearly before anyone enrolls.

Which states do you serve?

We’re based in Oviedo, Florida, and our advisors are licensed in multiple states, including Florida, Alabama, Indiana, Kentucky, Louisiana, Michigan, North Carolina, Ohio and Texas. We have taken care of businesses across the US, so ask us about your state.
